The FMC Assessment Data Tracking Sheet (PDF) is an optional companion to the FMC Self-Assessment.

Where the FMC Assessment is meant to provide a snapshot of how your body is supporting everyday movement at a specific point in time. The tracking sheet allows you to record these results so you can notice patterns, changes, or improvements when the assessment is completed again in the future. Thus, allowing you to be more proactive with your functional fitness.

This is a beta version of the assessment designed to help the user discover whether their current activity level is keeping them functionally fit. It is NOT fully digitized. You can either run the 6 pages that are the assessment, which you can fill out, or annotate the PDF using the text tools.
